Athletics

Parents/Guardians of student athletes intending to participate in Alameda High School athletics MUST complete this online registration process prior to engaging in any athletic activity.  Within 10 business days of submitting this registration form you will receive either an email stating that your athlete is clear or an email describing the changes that need to be made in order to be cleared.  

RETURNING ATHLETES: Please login to edit your account and register for the 2024-25 program.  You must upload a new physical, signed, dated, and stamped after July 1, 2024. 

NEW ATHLETES: Please create an account and register for the 2024-25 program.  You must upload a physical, signed and dated after July 1, 2024 .

INFORMATION YOU WILL NEED TO REGISTER YOUR STUDENT ATHLETE: 

• Physical Form must be dated July 1, 2024 or after 
  • A digital copy (image or scanned PDF) of your child’s Physical Form signed by a physician and a Parent/Guardian.  The form MUST also have a valid stamp from the doctor's office
• List of all sport(s) your student would like to try-out for
• Your student's date of enrollment at Alameda High School, ASTI, NEA, or ACLC
• Your student's Student ID #
• Contact information for at least 2 people, in case of emergency
• Your student’s physician's name and contact information
• Your health insurance information
• Name and dosage of medication your child may take

Athletic Approval will be granted after the Athletic Department verifies that all registration information is complete and a valid Physical Form is on file.  You will be notified by email of approval or of any missing or incomplete information.

NOTE - Although the program costs has a zero balance, please give strong consideration to our Athletic Booster request of $75.00 per sport per athlete.  Once you pay the $75 per season on the booster's website, the balance on ArbiterSports will remain zero; the accounts are not connected.   Sports programs may also ask for program specific donations.  This is at the discretion of the head coach.

Academic Eligibility – We expect our student-athletes to be students first and maintain exemplary character in the classroom by being in class on time everyday, modeling mature behavior and focusing on academics. If any student-athlete falls below a 2.0 he/she must see our Athletic Director for the possibility of being granted a one-time probation (only if GPA is between 1.50-1.99). If probation is granted, there will be weekly progress checks in order to maintain eligibility.

Transfer Eligibility – If your student has NOT attended Alameda High School for 12 months, he/she will need to see the Athletic Director for transfer paperwork. Transfer students are NOT allowed to participate in ANY game competition until cleared. Participation prior to clearance could result in forfeiture of games and ineligibility.

Instructions

1. Complete the PE Course Exemption Form entirely, leaving no blanks (includes student and parent’s/guardian’s signatures)
2. Submit the PE Course Exemption form to the Athletic Director to complete and sign. 
3. Submit the PE Course Exemption form to the Counseling Office, where someone will handle and monitor the remaining steps to process the PE Course Exemption.

FYI: The Counseling Office will make transcript changes ONLY after the following has been completed and documented.

• The Athletic Director completes, signs and dates the PE Course Exemption form
• The school's Principal completes, signs and dates the PE Course Exemption form  
• Educational Services staff (District Office) signs, dates, and returns the PE Course Exemption to the Counseling Office.

Once received, someone in the Counseling Office will input the change to your transcript.
